Why We’re Here
The Joey Bella Memorial Fund is a 501c3 charitable organization. We are non-sectarian and non-denominational. Founded to support children facing pediatric cancers, we have since expanded our mission to assist both children and parents impacted by other catastrophic illnesses. Below is a step-by-step guide of what you can expect during this process.
How to apply using our online form:
Step 1
Click to access our online application form. The form will ask for important information regarding:
The person completing the application and contact details
Patient information, including diagnosis and date of first diagnosis
Healthcare provider information
Description of financial need
Carefully review all details before submitting and make sure to certify the application requirements by checking the boxes next to each statement in the certification section.
Step 2
After submitting the form, gather the necessary requirements to complete your application, including:
Proof of home address (redacted license, lease, utility bill etc.)
Letter from treating physician confirming current diagnosis
(Note: Please download and use the provided template or identical information on office letterhead.)
Step 3
Email your documents: 1) Proof of home address and 2) Letter from treating physician to apply@joeybella.org. We will review your online application after all documents are received.

Gather the necessary requirements to complete your application, including:

1) Completed and signed application form
2) Proof of home address and
3) Letter from treating physician
